United Healthcare’s broad suite of fully insured innovative health insurance products offer a variety of health plans and plans design options with a range of employee choice, involvement and accountability.

LEVEL OF MEMBER INVOLVEMENT-LOW
Unitedhealthcare Choice plus Choice Plus gives members the freedom to see any doctor and visit any facility in our large nationwide network, including specialists,without a referral and without choosing a primary care doctor. If services are received outside of the network, costs will be higher. Choice Plus is a traditional health plan with copayments, coinsurance, deductibles and out-of-pocket maximums.

Unitedhealthcare Choice Choice allows members to see any network doctor,including specialists,or visit any network hospita, without needing a referral,however if services are delivered outside of our large national network, the plan will not cover the cost except in an emergency. While a primary care doctor is not mandatory, selecting one is recommended to help manage care. The plan also has copayments,coinsurance,deductibles and out-of-pocket maximums.

LEVEL OF MEMBER INVOLVEMENT-MODERATE
Unitedhealthcare Navigate Plus® Navigate Plus is a gated product, intended to deliver traditional benefits at a more budget-friendly price-point while still providing the plan design flexibility. Members choose a primary care physician, who will provide care for the majority of the member's health care needs, including preventive and routine care, and sickness or injury. Because the primary care physician will understand the member's health needs thoroughly, they can best refer the member to other network physicians when additional care is needed. The highest level of coverage is provided when the member has a referral from their primary care physician prior to receiving services from another network physician, and a lower level of coverage if there is no referral. The lowest level of coverage applies if a non-network provider is used.

LEVEL OF MEMBER INVOLVEMENT-HIGH
Unitedhealthcare Catalyst® Catalyst provides health care coverage by combining a comprehensive catastrophic plan with a limited pre-deductible upfront benefit allowance. The benefit allowance is used to help pay for certain eligible medical expenses before paying a deductible.

UnitedHealthCare Choice Plus HSA Available with Choice Plus and Choice, this consumer-driven health plan is made up of two parts: a high deductible health plan and a Health Savings Account(HSA). With these plans,you can open and deposit money, before taxes,into an HSA, which is your own personal savings account. Use the money to pay for eligible medical and pharmacy expenses, including your deductible, or save it for future expenses.

Plans that don’t break the bank

The number one concern for small-business owners is the cost of health care.1 So, All Savers Alternate Funding plans were built with your small business in mind. They’re intended to help save you money – and help your employees get more out of their plan, too.

Did you know that 60 percent of small-business employees spend less than $1,500 a year on health care?It’s often for things like ear infections, broken bones, or routine checkups. But since those employees don’t meet their deductibles, they end up covering their medical costs out of their own pocket year after year. All Savers Alternate Funding plans are different.

Coverage before the deductible

These plans were designed to help members pay for everyday health care costs before the deductible. With All Savers Alternate Funding plans, every enrolled employee and covered spouse gets $1,000 of up-front coverage, called a medical credit.The medical credit can be spent on most everyday health care costs.

A different kind of plan

All Savers Alternate Funding is a self-funded health plan designed specifically for small businesses. It includes three parts:

  1. Your self-funded medical plan, which pays covered medical expenses of your employees and theirdependents.
  2. A third-party administration agreement between you and United HealthCare Services, Inc. for claims processing, billing, customer service, and other administrative services.
  3. A stop-loss insurance policy by All Savers Insurance Company. Stop-loss insurance protects the plan from large, catastrophic claims by an individual covered member, and provides overall protection in the event that all claim payments made under the medical plan exceed a certain dollar limit

Why would you want a self-funded health plan? Because you’ll pay lower premium taxes throughout the year, your plan won’t be subject to state mandates, and you’ll actually have the chance to get some money back.

Life

Life insurance is vital to the financial security of your employees and their families. And we recognize this fundamental requirement by offering a variety of flexible life insurance plans. Use the buying power of your group to get affordable life insurance for your employees.

Basic Life Choose life coverage based on flat amounts, job level or a multiple of employee salary.
Basic plus dependent Add dependent life coverage for spouses and children to the basic plan
Basic plus supplemental Employees can choose to buy higher levels of coverage. Guaranteed issue amounts are available for life insurance and accidental death and dismemberment insurance based on participation and group size.
Basic, dependent and supplemental Employees can choose to add both dependent and supplemental insurance to their basic coverage
Accidental death and dismemberment(AD&D) AD&D provides coverage for loss of life or injuries incurred on or off the job and within 90 days from the date of an accident
